Unhinged isn’t really interested in social commentary. It’s just about delivering maximum violence with minimal fuss, nothing more.
It’s ironic that as the movie positioned to herald the return of major releases in theaters, Unhinged plays more like the kind of fare that’s gone straight to VOD in the last decade. Russell Crowe plays a role that Nicolas Cage or John Travolta might take on in one of those low-budget, low-profile thrillers, as an angry middle-aged man who goes on a killing spree following a traffic confrontation. Although Crowe’s nameless villain (listed only as “Man” in the closing credits) is already on a killing spree, really, since the movie opens with him murdering his ex-wife and her new partner/lover, and then setting their house on fire. Police are already searching for him when he’s stopped at a stop light in his giant pick-up truck in front of harried single mom Rachel (Caren Pistorius).
Late to drop off her son Kyle (Gabriel Bateman) at school, fired by her best hairdressing client and stuck in the middle of a nasty divorce, Rachel is having a really bad day, and she’s already spent far too much time stuck in traffic. Ellsworth’s screenplay for the 2005 Wes Craven thriller Red Eye worked in exactly that manner, but Unhinged director Derrick Borte is no Wes Craven, and the set -up here isn’t as clever or as skillfully executed. There’s some decent suspense during the movie’s handful of car chases — although those sequences grow repetitive pretty quickly — but the overall plot arc is completely predictable, and the man is just a violent brute force rather than a crafty or devious villain. There’s nothing connecting him to Rachel other than their chance encounter, and he’s not a threat on a psychological level, only as the equivalent of an escaped wild animal on a rampage.

At one point, the man forces Rachel to choose which of her friends or loved ones he’ll kill next, but that kind of moral dilemma is quickly tossed aside in favor of direct attacks, and Rachel never experiences any doubts or conflicts about how to respond to this psychopath. The police are almost immediately on her side, even though the man commandeers her cell phone and briefly passes himself off as her friend, and like far too many victims in thrillers like this, Rachel makes plenty of questionable decisions that prolong the danger she’s in. The dialogue is full of clumsy bits of foreshadowing that eventually pay off only in the most obvious, expected ways.
Without a complex plot or surprising twists, Unhinged really just relies on Crowe’s performance, and he certainly throws himself into the role in the way that Cage or Travolta often do in movies that don’t warrant so much effort. Crowe is clearly relishing the opportunity to play full-on evil, and he gives the man a hulking presence and speaks in an intermittent Southern accent, although the movie is set in a deliberately anonymous location. With his bulky figure, thick beard and hunched-over posture, Crowe often looks like he’s recording a motion-capture performance as a particularly mean gorilla. He can be entertaining to watch, especially when he goes eagerly over the top, but there’s nothing for him to play against.
Pistorius is completely overshadowed by Crowe’s outsize performance, and Rachel is nearly as thinly developed a character as the man is. She loves her son and her brother, and she’s stressed out about work and her divorce. That’s all the background the movie provides, and Pistorius’ performance doesn’t add any extra dimensions to it.
So there’s not much catharsis to seeing either main character get taken down, and the eventual resolution is both completely ridiculous and boringly predictable. As a VOD or streaming time-waster, Unhinged would be barely passable, but as a risky outing to a movie theater, it’s not even worth considering.
Starring Russell Crowe, Caren Pistorius, Gabriel Bateman, Austin P. McKenzie and Jimmi Simpson, Unhinged opens Friday, Aug. 21 in theaters nationwide.

Based on the cherished play Last Summer at Bluefish Cove by the late lesbian playwright Jane Chambers, Liz In September may change the location from Long Island Sound to the Caribbean, but Fina Torres’ film adaptation still maintains the heart and soul of the story. A group of friends gathers at the beach, knowing that it will be the last time they will be together with dying friend Liz (Patricia Velasquez). There is plenty of dyke drama, but there are also laughs and camaraderie. However, when a straight woman, on the run from her husband, joins the close-knit circle, and commands Liz’s attention, the mood of the gathering changes. Actress and model Patricia Velasquez stars as Liz in this three-hankie production (you’ve been warned) and gives the titular character considerable energy and spirit. I had the pleasure of speaking with Velasquez about the movie and more. [Liz In September will be available on DVD from Wolfe Video on November 3, 2015.]
Gregg Shapiro: You play the title character in the movie Liz In September. What are the challenges and rewards of being the lead in a movie?
Patricia Velasquez: If we start with the challenges, with this specific movie where there are a lot of women, it’s always a challenge because these are phenomenal actresses. Two of them, Mimi (Lazo) and Elba (Escobar) have been acting for many years. Elba has a big history and reputation in my country. It’s a little challenging when you’re the lead and you want everyone to feel the same. I found myself trying to make everyone feel equal. You feel like the weight of the film is on your shoulders. I studied Kabbalah for many years and I always try to look at the positive side. The glass is half full, not half empty. Playing the lead in a movie is challenging, but at the same time, the rewards are amazing. You have more of a say with the director, Fina Torres, in talking about all of the characters. You have the director’s ear to listen to what you have to say. I feel very fortunate.
GS: Have you ever had the opportunity to see a stage production of the play Last Summer at Bluefish Cove on which Liz In September is based?
PV: I haven’t seen a stage production of the whole play. I have seen a video. I belong to a Master Class in Los Angeles (taught) by an amazing acting coach named Ivana Chubbuck. I had been doing scenes from this play in class and I invited (director) Fina because we had been working together on a different project. Fina said to Ivana, “Patricia is doing a scene from Last Summer At Bluefish Cove? I can’t believe this! I tried to get this movie off the ground six years ago and we couldn’t.” When she saw me performing the scene, we changed projects. Fina decided to get the rights. While she was doing all that, I kept working on scenes from the play in class. It gave me such a deep connections to all of the characters, not just Liz. By the time Fina got the rights and decided to put the movie together, I knew these characters from the inside out.
GS: It’s interesting that Liz In September is the first movie version of a Jane Chambers play. What do you think it says that it’s a Venezuelan production and not an American one?
PV: They’ve been trying to get this specific play off the ground (for some time), but I can’t tell you what happened. It took someone like well-known director Fina Torres (to do it) who knew some of the people involved in getting the play adapted for film. She put all her energy and effort into doing it. For her adaptation of the play, what she did was add the vibe of the Latin people, the humor, the environment that she’s so famous for.
GS: Liz is a dog person and is very attached to Lola, her Golden Retriever. Are you also a dog person?
PV: If you go on my YouTube channel you will see a video of the Labrador that I had. The idea of having a dog in the film was because I have my house on Margarita Island (Venezuela) where I kept my dog that has now passed away. Fina came to Margarita Island. Retrievers are famous for their love of water. Every morning we’d wake up and go to the beach and the dog runs and runs. When I went in the water, she went in the water with me. Fina saw this for a couple of days and was fascinated by this relationship I had with my dog to the point that while we were sitting there, she said, “I really think we should have a dog in the film.” I said I would love it.
GS: Liz has a wonderful and loving circle of friends, including an ex with whom she is still close. Do you have a similar circle of friends?
PV: Yes, 100 per cent! I’m an Aquarius. One of the things about people born under the sign of Aquarius is that we live for our friends. I have a circle of friends in Venezuela that I speak to every day. That’s how close we are. I started the Wayúu Tayá Foundation for indigenous communities and everyone is involved. I am proud to say that I have a group of friends like that. By the way, some of the people in the movie are some of those friends! I’m very good friends with Mimi and Eloisa (Maturen) and Danay and Arlette. Making this movie made us very passionate about each other and about the project. This is a project that has never been done before; a movie about female gay characters has never been done before in the Hispanic world. We thought that we were doing something important. To see it resonate and be embraced within the LGBT community, and hopefully other communities, is wonderful. It made us think it was worth all the effort.
GS: One of the subjects that Liz In September addresses is assisted suicide. Where do you stand on the subject?
PV: I am all for it. We should have the power to make the decision about what we want to do with our lives. God forbid I am ever in that situation, but if I was I wouldn’t want to be a burden to other people. In that way, Liz and Patricia are very similar.
GS: Liz is also a limit-pusher. Are you, too?
PV: No. In that respect I’m not like Liz. If anything, I’m the opposite. One of the things we learn to do when training as actors is to never judge characters. You get to the point where when you don’t judge a character you are able to learn from them. I’m not a limit pusher. Honestly, I have to tell you I’m actually a very fearful person. Liz taught me to live a little more in the moment, but it’s still something I have to work on. That’s why acting is such an amazing experience, a learning process. It allows you to do things you would never do on your own.
GS: Liz falls for Eva, a woman who is married to a man. Have you ever fallen for a straight woman, and if so, how did it turn out?
PV: Yes, I can sincerely tell you that I have. One of the loves of my life was a married woman. I ran away [laughs] because I don’t like getting in the middle of relationships. She left her husband. For a long time she wanted something, but I wasn’t letting it happen. Then we were together for a while. But she had a really hard time accepting that she was gay. Or bisexual, I don’t know. She had a hard time in the society in which she lived and with her family. I was living in New York and I was open about the way I lived my life. Eventually it didn’t work out. Now I look back and I think sometimes we expect other people to do what we do. That’s a mistake. We have to give people the benefit of the doubt. Everyone tries their best and maybe didn’t see it because we live in a different world. It was a good lesson for me. Who was I to judge if she was doing the best she could, even if she wasn’t doing it the way I wanted.
GS: Finally, since we’re talking about movies, you recently published your memoir Straight Walk (Post Hill Press). If there was a movie version of Straight Walk who would you want to play you?
PV: You have no idea how many people have asked me that question [laughs]! The problem is that I’m too close to my book. I never wrote the book thinking that it could be a movie. Never for a minute! Now that the book is out, people keep asking me. I have to tell you that I don’t know.

Sharing her story anonymously, a 25-year-old pregnant woman took to Reddit to ask if she was the “a–hole” for being upset her husband, 28, is willing to skip out on what is supposed to be one of the most important days in their life together for a superhero movie.
“My husband and I are expecting a baby. As you may know, there is a new Batman movie releasing in early March,” the expectant mother wrote. “My husband is a big fan of that kind of stuff and wants to see it on opening day.”
Now, the couple has run into a little snag: “The issue is that our estimated due date is exactly on that day. I know that only a fraction of babies are actually born on the exact due date, but I have always been very regular on my periods and I have a feeling that I may be one of those cases.”
The reason the frustrated mom-to-be’s husband gave as to why he is so eager to see the new superhero flick? He wants to avoid spoilers. (It’s Batman and we kind of know how the story goes by now, so we’re not sure how many spoilers can be had, but whatevs…)
“He says it is important to see the movie on the first day because of spoilers. He said that even if I end up having the baby that day while he is watching the movie, at worst he would arrive a few hours late — which he says is not such a big deal.”
The man went a step further and accused his wife of being “irrational and emotional” due to being pregnant when she expressed her frustration over him prioritizing a movie over her and his baby.
The woman concluded by saying she’s ultimately “upset because I feel de-prioritized by him.”

The survey revealed that about one-third of middle- and high-school students said they were exposed to vaping aerosols in 2018 — an increase by about 30% compared with the previous three years, said Andy S.L. He called the increase in exposure to vaping aerosols “concerning,” given that an array of potentially hazardous chemicals are released by e-cigarettes. We caught up with the film’s up and coming stars to talk about the importance of coming of age stories, bonding on set and having a positive role model like Alita.
The Knockturnal: Alita is such a cool, fun movie, and you guys are an awesome crew in it. What kind of bonding experiences did you guys do to show that friendship on screen?
Jorge Lendeborg Jr.: I feel like we casually hung out, besides obviously going over the scenes together and then working together, which already is such a weird experience, because who else do you have to bond with? When you’re making a film it kind of feels like a summer camp. I guess the process of just making friends just kind of falls in line a bit faster.
Lana Condor: You know, you know what I remember is, we helped each other do auditions.
Jorge Lendeborg Jr.: Yeah, we did.
Lana Condor: During it … I might have done your Love, Simon audition.
Jorge Lendeborg Jr.: Yeah, we were both doing our Love, Simon auditions.
Lana Condor: And I remember like I just thought of this. We hung out, we bonded over Naked and Afraid, that reality show. Yeah, ’cause it’s crazy and they like go out and they’re like naked and afraid. And I showed him all my trashy reality television. Yeah, that’s how I forced them to be friends with me.
The Knockturnal: Smart. You guys were doing that awesome Rollerblade sport, did you guys actually get on the wheels and get going, or are you guys expert Rollerbladers now?
Keean Johnson: I can collectively say we’re not expert Rollerbladers. But you know, we did try, and I think that was another bonding experience, us being like “How cool are we?” And then the real skaters came on set the next day, and we were like “Oh, wow, we suck.” … I tried to go down a ramp at one point and almost broke my arm. Luckily, Jon, Robert or Jim didn’t see me do that.
Lana Condor: When Jon saw me on the Rollerblades for the first time, he told me to immediately take them off, because it looked like I was gonna break my face. So I just kind of sat that one out.
The Knockturnal: The film touches on different points like diversity, growing up. Talk about those messages and why it’s important for people to have those conversations.
Keean Johnson: Yeah, it has a lot to say about society as well, and how we kind of will just allow certain things to happen and look the other way. And I think that has a lot to say about what’s going on now. And then as far as young female empowerment, I think it’s great to have this young titular character who has so much heart isn’t just … Alita isn’t aggressive, Alita is defensive. And I think that’s why her fighting style is so interesting, and I think there’s gonna be a lot of young girls and young boys who will be testing their fighting skill in front of the mirror. Let’s hope they don’t smash their hand.
Lana Condor: The movie’s so great because it really does test and show whether or not you can stay a good person in maybe a bad environment. Because I think there are some bad things that happen in Iron City. It can be a savage place, but the fact that she comes out of it better than ever … it really does show that we can stay good in a bad environment. And that’s awesome, and I think that the world is so beautifully colored, and our movie’s so representative of what it really looks like out here in the real world, which is awesome. But yeah, and just, it reminds us to be empathetic and to be loving and to be curious, just like Alita is. And I think Alita is just the epitome of a role model for not just ladies, but also men and everyone. So I’m so excited for everyone to see it, ’cause I really think it’s gonna change a lot.

Dit jaar is het tien jaar geleden dat de Amerikaanse wetenschapsjournalist Gary Taubes met de arts-onderzoeker Peter Attia The Nutrition Science Initiative (NuSI) oprichtte. Met 40 miljoen dollar subsidie van een particuliere liefdadigheidsorganisatie gingen zij serieus onderzoek doen om te bewijzen dat hun theorie over de oorzaak van de obesitasepidemie klopte.
Tien jaar daarvoor, in 2002, schreef Taubes een artikel in The New York Times waarin hij stellig en overtuigend betoogde dat niet vet, maar koolhydraten in de voeding de oorzaak zijn van de uitdijende taille. Hij greep onder meer terug op het extreme laag-koolhydraat dieet dat de cardioloog Robert Atkins vijftig jaar geleden zowel beroemd en rijk als verguisd maakte.
In het dagelijks gebruik van velen werd Taubes’ theorie versimpeld tot het advies om geen of weinig koolhydraten te eten, omdat die door insuline worden omgezet in vet

In 2007 publiceerde Taubes zijn boek Good Calories, Bad Calories waarin hij zijn theorie uitwerkte en in een brede context plaatste. Het is nog altijd een indrukwekkende prestatie hoe Taubes op basis van honderden interviews en omvangrijk historisch onderzoek aantoont dat de voedingskundige aanbevelingen van die tijd (‘vet is slecht’) niet gebaseerd zijn op deugdelijke wetenschap, maar op politieke spelletjes en zakelijke belangen.
Als er één inspiratiebron voor de low carb-rage is die tot op heden woedt, dan is het dat boek. Taubes reisde de wereld over om zijn boek te promoten en zijn theorie te verkondigen en vond veel steun in de academische wereld. In het dagelijks gebruik van velen werd de theorie versimpeld tot het advies om geen of weinig koolhydraten te eten, omdat die door insuline worden omgezet in vet. Laagkoolhydraatdiëten worden door velen met succes gevolgd, maar daarmee is niet gezegd dat Taubes’ theorie klopt.
Experiment NuSI moest het harde wetenschappelijke bewijs leveren voor de ‘koolhydraat-insuline hypothese’. Taubes en Attia nodigden bekende en bekwame researchers uit om een opzet te maken voor een klinisch experiment met proefpersonen, die onder streng gecontroleerde omstandigheden verschillende diëten gevoerd zouden krijgen. Eén van de onderzoekers die Taubes aan zich bond was Kevin Hall, werkzaam bij het Amerikaanse National Institutes of Health.
Dat heeft Taubes geweten.
Hall vroeg en kreeg onvoorwaardelijke wetenschappelijke vrijheid voor het onderzoek. NuSI had geen invloed op het ontwerp van het voedingsexperiment en zou zich ook niet bemoeien met het interpreteren en presenteren van de uitkomsten.

De eerste maand kregen ze een streng ketogeen dieet, dus met amper koolhydraten, de tweede maand een dieet met veel koolhydraten.
De hoeveelheid calorieën en eiwitten werden gelijk gehouden in beide diëten. Volgens de theorie zouden proefpersonen op het koolhydraatdieet minder energie moeten verbruiken, omdat de hoge insulinesecretie vet opslaat en de vetverbranding remt. De uitkomst was echter anders: “Het koolhydraat-insulinemodel voorspelt een groter verlies van lichaamsvet tijdens de ketogeen-periode. Onze gegevens ondersteunen deze voorspelling niet.”
Kortlopend Een andere onderzoeker die door NuSI benaderd was, Stephan Guyenet, zegt op zijn blog dat hij aanvankelijk twijfel had over de onafhankelijke, objectieve aanpak van NuSI omdat Taubes en Attia wel erg verliefd waren op hun hypothese. Maar omdat er zwart op wit kwam te staan dat het onderzoek onder strikte omstandigheden gedaan zou worden, stemde hij in.
Op Guyenets blog is te lezen hoe het afliep. Kort samengevat: Hall deed een heel goed, doch kortlopend onderzoek en kon niet anders concluderen dan dat de hypothese van Taubes en Attia verworpen moest worden. Koolhydraten als nutriëntengroep in hun algemeenheid zijn niet dikmakend. Hij vond ook geen bewijs voor de aanname dat hoge insulineniveaus, veroorzaakt door koolhydraten (de zogenaamde glycemische lading) bovenmatig de vetopslag stimuleert.
De geldschieters stuurden geen cheques meer en NuSI verpieterde
Geldschieters
Die uitkomst beviel de mannen van NuSI niet, er moest een fout in het ontwerp van de test zitten. NuSI diskwalificeerde het eigen onderzoek. Er werd gesteggeld over de resultaten, er kwam ruzie. Attia vertrok in 2016, de geldschieters stuurden geen cheques meer en NuSI verpieterde. Hall ging bij zijn eigen instituut door met zijn veelgeprezen onderzoeken. Uit de hoek van liefhebbers van het ketogene dieet kwam flink wat commentaar op het onderzoek. Een maand was veel te kort van duur geweest, er werden nog te veel koolhydraten gegeten om echt effectief te zijn en Hall bagatelliseerde de geringe verbeteringen die toch te zien waren in de metingen.
Niettemin kwam ook uit andere richtingen, onder meer van het Israëlische Weizmann-instituut, bewijs dat het koolhydraat-insuline model op losse schroeven zette. Het onwrikbare verband tussen koolhydraten in het eten en obesitas, met als tussenstap een opgejaagde insulineproductie, zoals Taubes die stipuleerde, bestaat niet in een ‘normale’ dagelijkse voedingspraktijk.
Andere gerespecteerde wetenschappers, onder wie David Ludwig, Arne Astrup. Cara Ebbeling en Jeff Volek, bleven volharden in het CIM, het carbohydrate insulin model. Dat geeft volgens hen een betere verklaring voor de metabole processen die tot overgewicht leiden dan het EBM, het energy balance model, dat zegt dat te veel eten van àlles de oorzaak is.
We worden niet dik omdat we eten, we eten omdat we dik worden
Omgekeerde causaliteit
Eind vorig jaar publiceerden zeventien low carb-zwaargewichten, onder wie de hierboven genoemde, een verdediging van hun ietsje aangepaste CIM. Foodlog schreef daarover onder de kop: ‘Low carb strikes back’. Niet uitsluitend koolhydraten zorgen voor verhoogde insulinesecretie, het gaat vooral om voeding met een hoge glycemische lading (een concept dat zijn eigen zwakheden kent).
Maar verder bleef het model intact: niet de hoeveelheid calorieën, maar de kwaliteit van de voeding stuurt het vetmetabolisme. En belangrijk in de hypothese: de omgekeerde causaliteit. We worden niet dik omdat we eten, we eten omdat we dik worden. Door de koolhydraten en de insuline krijgen we alleen maar meer honger, terwijl er minder vet wordt verbrand.
Energiebalansmodel Het kon niet uitblijven natuurlijk: Hall en de anderen kwamen met een antwoord op de CIM-aanval op ‘hun’ EBM. Niet in de vorm van nieuw, gedegen onderzoek, maar met een weerlegging op basis van relevante en recente wetenschappelijke literatuur, waaronder die van zichzelf. Het moet gezegd, hun stuk ‘The energy balance model of obesity: beyond calories in, calories out’ is heel overtuigend.

Het tweede deel van de titel zal voor de oplettende lezer mogelijk wat vreemd lijken. Want het energiebalansmodel is toch gebaseerd op het gegeven van ‘calories-in-is-calories-out’? Elk pondje gaat door het mondje, energie kan niet verloren gaan, zoals de Eerste Hoofdwet van de Thermodynamica voorschrijft?
Stroman-argumentatie Hall en zijn co-auteurs openen hun weerwoord met de constatering dat de EBM wel iets meer omvat en dat de CIM-aanval een karikatuur van EBM maakte, en daarmee een stroman-argumentatie was. “Het EBM gaat ervan uit dat de hersenen het belangrijkste orgaan zijn dat verantwoordelijk is voor de regulering van het lichaamsgewicht via complexe endocriene, metabolische en neurologische signalen waarmee de voedselinname wordt gecontroleerd,” schrijven ze. Die signalen zijn actief “onder het niveau van ons bewustzijn” en reageren op “invloeden van de omgeving zowel als op de energiebehoefte van het lichaam.”
Hall en zijn co-auteurs gaan het rijtje invloeden af en kijken waar het CIM en de aanval op het EBM mank gaan. Er is niet sprake van één hormoon (insuline) dat vanuit één orgaan (het vetweefsel) één nutriënt (koolhydraten) reguleert. Studies met knaagdieren, genetische studies bij de mens, voedingsexperimenten met proefpersonen èn interventies met medicijnen leveren een “overweldigende hoeveelheid bewijsmateriaal [dat erop wijst] dat talrijke variabelen in de voedselomgeving naast hoge glycemische voedingsmiddelen kunnen leiden tot een verhoogde energie-inname. De EBM stelt dat obesitas kan ontstaan als een of meer van deze factoren in het spel zijn.”
Ondanks de pittige, kritische toon van het debat lijken de theorieën toch naar elkaar toe te kruipen

Het EBM is “stevig gebaseerd op biologische mechanismen” en niet een dogmatische herhaling van de natuurwetten over energiebehoud, wat de critici suggereren. “Verdere ontwikkeling van het EBM vereist opheldering over de factoren in de dynamische voedselomgeving die het meest verantwoordelijk zijn voor het ontstaan van obesitas, de mechanismen waardoor deze factoren de hersencircuits die de voedselinname controleren veranderen, en waarom sommige individuen vatbaarder zijn voor ontwikkeling van obesitas dan anderen.” Dit sluit aan bij recent hersenonderzoek naar de gebieden waarin trek en verzadiging, energie-inname en andere delen van het metabolisme geregeld worden.
Blijft er nog iets heel van het CIM? Ondanks de pittige, kritische toon van het debat lijken de theorieën toch naar elkaar toe te kruipen. De regulering van alle processen die met eten te maken hebben worden aangestuurd vanuit de hersenen, zijn complexer dan voorheen gedacht, en zijn sterk individueel bepaald. Hall c.s. zeggen tussen de regels min of meer dat het nieuwe CIM goedbeschouwd een onderdeel is van het bredere EBM. Maar ook schreef Hall eerder, in zijn NuSI-studie, toegeeflijk: “Het is (…) mogelijk dat koolhydraatbeperking kan leiden tot een verminderde ad libitum energie-inname – een voorspelling van het koolhydraat-insulinemodel die in de huidige studie niet werd getest, maar die nader onderzoek verdient.”
Dat onderzoek is er al: de gedocumenteerde ervaringen van al die mensen die een laagkoolhydraatdieet weten vol te houden, afvallen, metabool gezonder worden en zelfs van hun diabetes afkomen.

Laagkoolhydraatdiëten worden door velen met succes gevolgd, maar daarmee is niet gezegd dat Taubes’ theorie klopt.
Experiment NuSI moest het harde wetenschappelijke bewijs leveren voor de ‘koolhydraat-insuline hypothese’. Taubes en Attia nodigden bekende en bekwame researchers uit om een opzet te maken voor een klinisch experiment met proefpersonen, die onder streng gecontroleerde omstandigheden verschillende diëten gevoerd zouden krijgen. Eén van de onderzoekers die Taubes aan zich bond was Kevin Hall, werkzaam bij het Amerikaanse National Institutes of Health.
Dat heeft Taubes geweten.
Hall vroeg en kreeg onvoorwaardelijke wetenschappelijke vrijheid voor het onderzoek. NuSI had geen invloed op het ontwerp van het voedingsexperiment en zou zich ook niet bemoeien met het interpreteren en presenteren van de uitkomsten.

De eerste maand kregen ze een streng ketogeen dieet, dus met amper koolhydraten, de tweede maand een dieet met veel koolhydraten.
De hoeveelheid calorieën en eiwitten werden gelijk gehouden in beide diëten. Volgens de theorie zouden proefpersonen op het koolhydraatdieet minder energie moeten verbruiken, omdat de hoge insulinesecretie vet opslaat en de vetverbranding remt. De uitkomst was echter anders: “Het koolhydraat-insulinemodel voorspelt een groter verlies van lichaamsvet tijdens de ketogeen-periode. Onze gegevens ondersteunen deze voorspelling niet.”
Kortlopend Een andere onderzoeker die door NuSI benaderd was, Stephan Guyenet, zegt op zijn blog dat hij aanvankelijk twijfel had over de onafhankelijke, objectieve aanpak van NuSI omdat Taubes en Attia wel erg verliefd waren op hun hypothese. Maar omdat er zwart op wit kwam te staan dat het onderzoek onder strikte omstandigheden gedaan zou worden, stemde hij in.
Op Guyenets blog is te lezen hoe het afliep. Kort samengevat: Hall deed een heel goed, doch kortlopend onderzoek en kon niet anders concluderen dan dat de hypothese van Taubes en Attia verworpen moest worden. Koolhydraten als nutriëntengroep in hun algemeenheid zijn niet dikmakend. Hij vond ook geen bewijs voor de aanname dat hoge insulineniveaus, veroorzaakt door koolhydraten (de zogenaamde glycemische lading) bovenmatig de vetopslag stimuleert.
De geldschieters stuurden geen cheques meer en NuSI verpieterde

Die uitkomst beviel de mannen van NuSI niet, er moest een fout in het ontwerp van de test zitten. NuSI diskwalificeerde het eigen onderzoek. Er werd gesteggeld over de resultaten, er kwam ruzie. Attia vertrok in 2016, de geldschieters stuurden geen cheques meer en NuSI verpieterde. Hall ging bij zijn eigen instituut door met zijn veelgeprezen onderzoeken. Uit de hoek van liefhebbers van het ketogene dieet kwam flink wat commentaar op het onderzoek. Een maand was veel te kort van duur geweest, er werden nog te veel koolhydraten gegeten om echt effectief te zijn en Hall bagatelliseerde de geringe verbeteringen die toch te zien waren in de metingen.
Niettemin kwam ook uit andere richtingen, onder meer van het Israëlische Weizmann-instituut, bewijs dat het koolhydraat-insuline model op losse schroeven zette. Het onwrikbare verband tussen koolhydraten in het eten en obesitas, met als tussenstap een opgejaagde insulineproductie, zoals Taubes die stipuleerde, bestaat niet in een ‘normale’ dagelijkse voedingspraktijk.
Andere gerespecteerde wetenschappers, onder wie David Ludwig, Arne Astrup. Cara Ebbeling en Jeff Volek, bleven volharden in het CIM, het carbohydrate insulin model. Dat geeft volgens hen een betere verklaring voor de metabole processen die tot overgewicht leiden dan het EBM, het energy balance model, dat zegt dat te veel eten van àlles de oorzaak is.

Omgekeerde causaliteit
Eind vorig jaar publiceerden zeventien low carb-zwaargewichten, onder wie de hierboven genoemde, een verdediging van hun ietsje aangepaste CIM. Foodlog schreef daarover onder de kop: ‘Low carb strikes back’. Niet uitsluitend koolhydraten zorgen voor verhoogde insulinesecretie, het gaat vooral om voeding met een hoge glycemische lading (een concept dat zijn eigen zwakheden kent).
Maar verder bleef het model intact: niet de hoeveelheid calorieën, maar de kwaliteit van de voeding stuurt het vetmetabolisme. En belangrijk in de hypothese: de omgekeerde causaliteit. We worden niet dik omdat we eten, we eten omdat we dik worden. Door de koolhydraten en de insuline krijgen we alleen maar meer honger, terwijl er minder vet wordt verbrand.
Energiebalansmodel Het kon niet uitblijven natuurlijk: Hall en de anderen kwamen met een antwoord op de CIM-aanval op ‘hun’ EBM. Niet in de vorm van nieuw, gedegen onderzoek, maar met een weerlegging op basis van relevante en recente wetenschappelijke literatuur, waaronder die van zichzelf. Het moet gezegd, hun stuk ‘The energy balance model of obesity: beyond calories in, calories out’ is heel overtuigend.

Het tweede deel van de titel zal voor de oplettende lezer mogelijk wat vreemd lijken. Want het energiebalansmodel is toch gebaseerd op het gegeven van ‘calories-in-is-calories-out’? Elk pondje gaat door het mondje, energie kan niet verloren gaan, zoals de Eerste Hoofdwet van de Thermodynamica voorschrijft?
Stroman-argumentatie Hall en zijn co-auteurs openen hun weerwoord met de constatering dat de EBM wel iets meer omvat en dat de CIM-aanval een karikatuur van EBM maakte, en daarmee een stroman-argumentatie was. “Het EBM gaat ervan uit dat de hersenen het belangrijkste orgaan zijn dat verantwoordelijk is voor de regulering van het lichaamsgewicht via complexe endocriene, metabolische en neurologische signalen waarmee de voedselinname wordt gecontroleerd,” schrijven ze. Die signalen zijn actief “onder het niveau van ons bewustzijn” en reageren op “invloeden van de omgeving zowel als op de energiebehoefte van het lichaam.”
Hall en zijn co-auteurs gaan het rijtje invloeden af en kijken waar het CIM en de aanval op het EBM mank gaan. Er is niet sprake van één hormoon (insuline) dat vanuit één orgaan (het vetweefsel) één nutriënt (koolhydraten) reguleert. Studies met knaagdieren, genetische studies bij de mens, voedingsexperimenten met proefpersonen èn interventies met medicijnen leveren een “overweldigende hoeveelheid bewijsmateriaal [dat erop wijst] dat talrijke variabelen in de voedselomgeving naast hoge glycemische voedingsmiddelen kunnen leiden tot een verhoogde energie-inname. De EBM stelt dat obesitas kan ontstaan als een of meer van deze factoren in het spel zijn.”
Ondanks de pittige, kritische toon van het debat lijken de theorieën toch naar elkaar toe te kruipen

Het EBM is “stevig gebaseerd op biologische mechanismen” en niet een dogmatische herhaling van de natuurwetten over energiebehoud, wat de critici suggereren. “Verdere ontwikkeling van het EBM vereist opheldering over de factoren in de dynamische voedselomgeving die het meest verantwoordelijk zijn voor het ontstaan van obesitas, de mechanismen waardoor deze factoren de hersencircuits die de voedselinname controleren veranderen, en waarom sommige individuen vatbaarder zijn voor ontwikkeling van obesitas dan anderen.” Dit sluit aan bij recent hersenonderzoek naar de gebieden waarin trek en verzadiging, energie-inname en andere delen van het metabolisme geregeld worden.
Blijft er nog iets heel van het CIM? Ondanks de pittige, kritische toon van het debat lijken de theorieën toch naar elkaar toe te kruipen. De regulering van alle processen die met eten te maken hebben worden aangestuurd vanuit de hersenen, zijn complexer dan voorheen gedacht, en zijn sterk individueel bepaald. Hall c.s. zeggen tussen de regels min of meer dat het nieuwe CIM goedbeschouwd een onderdeel is van het bredere EBM. Maar ook schreef Hall eerder, in zijn NuSI-studie, toegeeflijk: “Het is (…) mogelijk dat koolhydraatbeperking kan leiden tot een verminderde ad libitum energie-inname – een voorspelling van het koolhydraat-insulinemodel die in de huidige studie niet werd getest, maar die nader onderzoek verdient.”
Dat onderzoek is er al: de gedocumenteerde ervaringen van al die mensen die een laagkoolhydraatdieet weten vol te houden, afvallen, metabool gezonder worden en zelfs van hun diabetes afkomen.
